Wait at least 3 months after the first treatment to have your second treatment then mail your Love It Rebate Form and treatment receipt (again credit card receipts not accepted) within 30 days of your second treatment and receive your rebate check. If for some reason you don’t love your Dysport™ treatment then within 15-30 days after your first treatment with Dysport™ go to www.dysportusa.com and click on print forms and select the Leave It option and print out your Leave It Rebate Form. Wait 3 months then get treated with another botulinum toxin type A product and mail in your rebate form along with your treatment receipt within 30 days of the second treatment.
